In a nutshell, no – you don’t apply for NHS Continuing Healthcare in the usual sense. In other words, you don’t have to fill in an application form.

Instead, the process should be automatic.

The Checklist assessment is the first stage of the NHS Continuing Healthcare process, and it should be carried out automatically by a health or social care professional when your relative needs care. This could be a social worker, district nurse, hospital discharge team, etc.

However, this doesn’t always happen, so you may have to ask for a Checklist.
